Request MyChart proxy access

Parents or guardians can request access to a minor child by logging into their own MyChart account and navigating to “Sharing options.”

Note: MyChart will ask for some information before sending your request for review and approval.

You can give someone else access to your MyChart. This can be done through the “Share My Record” option under “Sharing.” Once you’re on the “Share My Record” screen, select the “Friends and family access” option.

Note: You’ll need to provide the email address of the person you’d like to have proxy access to your chart.

To request access, complete one of the forms below. Completed forms can be submitted in-person to your primary care clinic (North – Robbinsdale Hospital or North – Maple Grove Hospital registration team), or mailed to:
