Survey to Serve as Important Part of 2013 Community Health Needs Assessment
In an effort to better meet the health needs of our community, North Memorial Medical Center is conducting a community health needs assessment. As part of that effort, we are conducting a survey to identify unmet health care needs in our community. Your feedback will help us better serve our surrounding neighborhoods.
