Referrals for home health services can be made by physicians, hospital and nursing home discharge planners, assisted living staff, social workers, family members, friends, patients themselves and other agencies. North Memorial Home Health provides skilled services designed for short-term care and private duty for chronic care support. Our staff is respectful of and honors patient and family choices, care giving and decision-making. Services range from: skilled nursing/therapies to light housekeeping and meal preparation. Our rehabilitation specialists bring expertise along with the most recent technology to treat weakness, discomfort, neuropathy and lymphedema while emphasizing safety.

Palliative Home Health Care serves individuals with advanced or serious diseases (such as cancer, congestive heart failure, or emphysema). The goal of palliative care is to help individuals experience the best possible quality of life regardless of the need for other medical care. Home visits are provided to you by a Nurse, Social Worker, Home Health Aide or Therapist who work with your physician to:
Manage distressing symptoms
Enhance support for patient and family
Assist with planning for ongoing care and needs
Palliative care is provided throughout the course of an illness and can be offered at any stage in any illness.
Guidelines for Admission to North Memorial Home Health Skilled Services
Signed physician's order
Patient requires skilled nursing care
Patient is home-bound or experiences a very "taxing effort" to leave home
Patient may be receiving TPN, chemotherapy, radiation, dialysis, transfusions, IVs or wound care
Patient needs teaching of a disease process that has caused recurrent hospitalizations and medication management

Medicare, Medical Assistance, most long term care plans and private insurance plans cover many home care services that a physician has ordered. Private pay options are also available. Private Duty
North Memorial Home Health offers Private Duty service to seniors or people with disabling conditions, who wish to remain in their own homes and can do so with a little help. By providing supportive home care services and care management coordination of needs, we can extend the time one can safely remain at home.
Who Can Benefit from Private Duty Services?
Those individuals who:
Have recently discharged from the hospital or skilled nursing facility who require assistance until strength returns
Require medication management
Need assistance with personal care (dressing, grooming, bathing)
Have diminished eyesight or hearing
Are frail and need assistance to remain in their home
Have adult children living out of state or who are not able to provide or coordinate care on a daily basis
A physician's order is required and can be obtained by our admissions team. Our nurses will contact the physician to discuss the patient's needs. Referrals can be made by the: prospective patient, family members, friends and health care providers. Payment is private pay, through some individual insurance providers, or long term care insurance. Medicare does not cover these services.

Getting Started
A Registered Nurse Case Manager will assess the patient's needs to determine the appropriate amount and frequency of care required. The nurse works with the patient, family and physician to develop a personalized plan of care. These services are most often private pay, but can also be covered by third aprty payers such as long term care insurance, county funds, Veteran's Administration or others when eligible.
