If you are preparing for a hospital stay or a visit, we want your visit to be as pleasant as possible. In order to address questions about our campus and surrounding area, we have created many services to help you while you are here.

Contacting a Patient
If you would like to contact a patient at North Memorial, please call our patient information line: 763-520-5100.

Medical Records
To obtain hospital medical records from North Memorial Medical Center or Maple Grove Hospital, or to release them to another provider, please complete an authorization form, sign and return it one of two ways:

  • Fax it to 763-520-4271 (our alternate fax number is 763-520-4730) or
  • Mail your signed form to:

To release records from a North Memorial-owned clinic, including vaccination records, you may use this same form. Complete, sign and return it by fax or mail to your clinic. The contact information for all North Memorial Clinics is available at http://northmemorial.com/NMClinics. Affiliated clinics are not owned or operated by North Memorial and must be contacted directly for instructions on how to release your records.
