North Memorial Sleep Care Services (PAP supplies and equipment), located in Maple Grove and Robbinsdale, is no longer providing any new services.

North Memorial Sleep Health Centers remain open and continue to provide sleep diagnostic services in Robbinsdale and Maple Grove.

• Please contact your sleep specialist physician's office to assist you with the transfer your prescription information to another durable medical equipment provider.

• For your convenience, effective January 28, 2013 all PAP compliance downloads will be completed at your office appointment with your sleep physician.
• If you are currently under a rental agreement with North Memorial Sleep Care Services, you will continue to receive a billing statement from us during the close of business transition.
• If you are still within the first 90 days of your PAP setup and require assistance, please call 763-581-5063. Leave a message and we will return your call.
