If you need to speak to a physician after clinic hours, please call (763) 257-8000. Our phones are answered at all times, and you will be referred to the on-call physician. In case of an emergency, call 911.

North Memorial Clinic accepts most major health care insurance plans and we will file your insurance claims for you. Please bring your insurance card each time you visit.

Our Doctors

 

Luke Hammer, PA-C

Luke Hammer, PA-C is a physician assistant in Family Practice and Urgent Care. His special areas of interest include healthy lifestyle promotion, disease prevention, pediatric and adolescent health concerns and chronic health condition management.
Mr. Hammer received his Bachelor of Arts degree in Biology from St. Olaf College. He completed his Master of Physician Studies at the University of Iowa. Hammer is board certified by the National Commission of Certification of Physician Assistants and is licensed by the Minnesota Board of Medical Practice. His affiliations include the American and Minnesota Academies of Physician Assistants.
In his free time, he enjoys spending time with family at their northern Minnesota cabin and is active in cycling, basketball, golf and running.
 

Jim Guyn, MD

Jim Guyn is a Family Practice physician with interests in adolescent medicine, pediatrics, obstetrics and gynecology, preventive medicine, and sports medicine. Dr. Guyn received his medical degree from the UCLA School of Medicine. In 1982, he completed his Family Practice residency at Ventura County Medical Center, California. He is board certified by the American Board of Family Practice.
 

Verna Hillstrom, MD

Verna Hillstrom, MD is a graduate of the Ohio State University. She went on to receive her medical degree from the Ohio State University College of Medicine. She completed her internship and residency at Hennepin County Medical Center. She is board certified in family medicine and is a member of the American and Minnesota Academies of Family Practice.
Her special interests are pediatrics and adolescence.
In her spare time, Dr. Hillstrom enjoys gardening, reading and missions work.
 

Wanda Andrews, PA-C

Wanda Andrews is a Physician Assistant in Family Practice. She received her bachelor's degree in Physician Assistant Studies from Augsburg College in Minneapolis, and has an AS degree in Dental Hygiene from Mankato State University. Wanda is a board certified Physician Assistant by the National Commission of Certification of Physician Assistants. Her professional affiliations include the American Academy of Physician Assistants, the Minnesota Academy of Physician Assistants, and she is a Physician Assistant advisor for the Minnesota Health Care Reform Review Council.
Her clinical areas of interest are diabetes, chronic care, women's health and dermatology.
In her spare time she enjoys cooking, golf and participates in the Susan G. Komen 3-day run.
She comes to Elk River Clinic after 14 years in family medicine at North Memorial Clinic - Minnetonka Physicians.
 

Mark Zelent, DPM

Mark Zelent, DPM specializes in diseases and disorders of the foot and ankle. He completed his education at the University of Illinois. Dr. Zelent did an internship at the Veteran's Medical Center in Minneapolis and his residency in Foot and Ankle Surgery at Mercy Hospital, Coon Rapids, where he served as chief resident. He also had the opportunity to complete Orthopedic Reconstructive Surgery Fellowship experiences at Harborview Medical Center in Seattle Washington and at the Russian Ilizarov Scientific Center for Restorative Traumatology and Orthopedics.
Dr. Zelent is board certified in Foot Surgery and Reconstructive Rearfoot and Ankle Surgery by the American Board of Podiatric Surgery. He is also board certified by the American Board of Orthopedics and Primary Podiatric Medicine for non-surgical management.
Dr. Zelent has been an author of numerous publications and currently serves as a manuscript reviewer for several medical journals. He enjoys spending time with family and his hobbies include cooking and baking. He also participates as a recipe reviewer for a prominent culinary publication.
